Background: Harlequin color change is a benign, idiopathic, self-limiting disorder characterized by an appearance of skin divided into two distinctly colored areas. Its etiology unknown but thought to be caused immaturity hypothalamic regulation peripheral vascular tone. COVID-19 infection in neonates infrequent and rarely symptomatic, with only few cases described the literature. In isolation, both conditions have low incidence. This first case reported world literature harlequin newborn who tested positive for COVID-19. There isn’t single publication that links COVID-19, yet. The Case: Newborn 34-year old mother. Who at 3 hours life presented progressive respiratory distress whimpering cyanosis. At 20 sudden right hemibody unilateral hyperemia contralateral pallor, simultaneous demarcated line separating left sides body, involving trunk, extremities face, this remained 10 minutes then began fade on its own until it disappeared completely. were no changes vital parameters associated phenomenon.
